SYSTEMATIC REVIEW AND META-ANALYSIS : THE EFFECT OF FUROSEMID TO IMPROVE OUTCOME ACUTE KIDNEY INJURY IN SEPSIS AND SEPSIS SHOCK

Background: Sepsis is very closely related to Acute Kidney Injury (AKI) related to increased kidney oxygen demand. Furosemide is an alternative therapy in patients with sepsis because it can reduce oxygen demand in the kidneys. Early diagnosis and initial therapy are linear with mortality. The use of furosemide is still sufficient. controversy considering that one of the main problems in sepsis is fluid dysregulation. Consider that the reference for treatment is not sufficient, it is therefore necessary to carry out further research to determine the effectiveness of furosemide against AKI

Objective: This study aims to determine the effect of Furosemid to improve putcome acute kidney injury in sepsis and sepsis shock

Methods: Systematic review and meta-analysis, 721 journals from 3 databases were analyzed use the PRISMA guidelines so that 9 journals were obtained and then analyzed use a forest plot to show correlation by looking at some of the AKI outcomes.

Result: The use of furosemide compared to the control resulted in a higher urine output value, (P = 0.05), a more negative fluid balance of 25.43 mL, a lower urea value of 2.74 mg/dL, a decrease in creatinine and an increase in creatinine which were not significant, blood levels Ngal 0.51 lower, significant renal improvement in patients with AKI who experienced septic shock p <= 0.05, p = 0.03.

Conclusion: The administration of furosemide was able to significantly reduce AKI compared to controls
