Background: An impacted tooth is defined as a tooth that fails to erupt completely into its functional position due to insufficient space in the dental arch or obstruction by adjacent teeth or pathological tissues. Based on incidence rates, the posterior teeth most frequently affected by impaction are the mandibular and maxillary third molars. The Pell and Gregory classification evaluates the relationship of the tooth to the mandibular ramus and the depth of the molar impaction within the mandibular bone. While dental impaction can manifest symptoms ranging from mild to severe, not all impaction cases result in significant clinical complications. The Visual Analogue Scale (VAS) is regarded as the most efficient instrument for assessing patient pain levels. This linear scale provides a visual representation of the pain intensity experienced by the patient. Objective: To determine the correlation between the classification of mandibular third molar impaction based on panoramic radiography and the pain scale at Dr. Moewardi Hospital during the 2023-2024 period. Methods: This study employed an observational analytic research design utilizing secondary data from medical records and Digital Imaging and Communications in Medicine (DICOM) files derived from patients' panoramic examinationsResults: Statistical analysis of the relationship between Pell and Gregory Classes I, II, and III and the pain scale yielded a p-value <0.001. Furthermore, the analysis of the relationship between Levels A, B, and C and the pain scale showed a p-value of 0.002.Conclusion: The results indicate a statistically significant correlation between the Pell and Gregory classification and the VAS pain scale. With a correlation coefficient of r=0.190, the strength of this relationship is categorized as very weak with a positive direction. Keyword: Panoramic, Impaction, Mandibular Third Molar, Pain Scale
