Pendahuluan: Penyakit paru obstruksi kronis (PPOK)
adalah penyebab kematian ketiga di dunia. Salah satu penyebab utama PPOK adalah
kebiasaan merokok. Eksaserbasi PPOK secara jelas dapat mengganggu pertukaran
gas, sehingga PaCO₂ memiliki pengaruh yang besar terhadap status asam-basa
tubuh. Hubungan antara derajat merokok dan gambaran foto toraks paru emfisema
terhadap kadar PaCO₂ masih perlu diteliti lebih lanjut.
Metode: Penelitian ini merupakan penelitian observasional analitik
dengan rancangan retrospektif. Data dikumpulkan melalui pengukuran indeks
brinkman, analisis foto toraks, dan analisis gas darah dari pasien PPOK
eksaserbasi akut di RSUD Dr. Moewardi. Data didapatkan pada periode Januari
2020-November 2024 sebanyak 50 sampel. Sampel ditetapkan berdasarkan kriteria
inklusi dan eksklusi dengan cara purposive sampling. Analisis bivariat
menggunakan uji Chi-Square.
Hasil: Analisis menggunakan uji Chi-Square antara gambaran
foto toraks paru emfisema dengan kadar PaCO₂ darah pasien PPOK eksaserbasi akut
di RSUD Dr. Moewardi didapatkan hasil signifikansi p<0>Chi-Square
antara derajat merokok dengan kadar PaCO₂ darah pasien PPOK eksaserbasi akut di
RSUD Dr. Moewardi didapatkan hasil tidak signifikan (p=0,052).
Simpulan:
Terdapat
hubungan yang signifikan antara gambaran foto toraks emfisema dengan kadar
PaCO₂ darah pada pasien PPOK eksaserbasi akut. Namun, tidak terdapat hubungan
yang signifikan antara derajat merokok dengan kadar PaCO₂ darah pasien PPOK
eksaserbasi akut di RSUD Dr. Moewardi Surakarta.

Introduction: Chronic obstructive pulmonary disease
(COPD) is the third leading cause of death in the world. One of the main causes
of COPD is smoking. Exacerbations of COPD can clearly interfere with gas
exchange, so that PaCO₂ has a major influence on the body's acid-base status.
The relationship between smoking degree and chest X-ray imaging of emphysema
lungs with PaCO₂ levels still needs further study.
Method: This study is an analytical
observational study with a retrospective design. Data were collected through
Brinkman index measurements, chest X-ray analysis, and blood gas analysis from
patients with acute exacerbation of COPD at Dr. Moewardi Hospital. Data were
obtained in the period January 2020-November 2024 as many as 50 samples. The
sample was determined based on inclusion and exclusion criteria by purposive
sampling. Bivariate analysis using the Chi-Square test.
Result: Analysis using the Chi-Square test between the chest X-ray of emphysema lungs and blood PaCO₂ levels of acute exacerbation COPD patients at Dr. Moewardi Hospital obtained a significance result of p<0 p=0.052).>
Conclusion:
There is a
significant relationship between chest X-ray images of emphysema and blood
PaCO₂ levels in patients with acute exacerbation of COPD. However, there is no
significant relationship between the degree of smoking and blood PaCO₂ levels
in patients with acute exacerbation of COPD at Dr. Moewardi Hospital Surakarta.
