The increasing threat of cyber incidents within government institutions necessitates the development of a documented, structured, and accessible online information security incident reporting system. Until now, the reporting process at the Department of Communication, Informatics, Statistics, and Encryption (Diskominfo SP) Surakarta has been conducted manually, making it difficult to track reports, coordinate responses, and make informed decisions. To address this issue, a website-based information system was developed to facilitate the submission of security incident reports by government agencies (OPD). The system was designed using the waterfall methodology, which includes the stages of requirements analysis, system design, implementation, and testing. The core features of the system include incident submission by OPD administrators, response handling by Diskominfo superadmins, and a digital follow-up and tracking mechanism for submitted reports. Test results show that the system performs in accordance with user needs and successfully improves coordination efficiency while accelerating the handling of incidents. This system serves as a strategic solution for managing information security incidents within Diskominfo SP Surakarta. In the future, the system could be further enhanced with automatic notification features and integration with national cybersecurity platforms.
