Scope:
The
Continuity of Care (CoC) was extended to Mrs. N comprehensively from gestation,
delivery, post-parturition, and neonate to family planning program in
accordance with the existing midwifery care standards. 
Implementation:
The antenatal care was extended to Mrs. N with obesity and intrauterine device
(IUD) in situ. The delivery was performed with C-section. Early breastfeeding
initiation was not done. The neonatal care went on normally, and the infant was
given formula milk. The post-partum care went on normally and was performed in
three visits. The family planning program took place normally. 
Evaluation:
The antenatal care on the pregnancy with obesity and IUD in situ went on
normally. The delivery was done with C-section. The neonatal care happened
normally, and the formula milk was administered to the infant.  The mother was given post-parturition-related
health education. The mother chose the tubal ligation contraceptive for her
family planning program.
Conclusion
and Recommendation: No gap was found during the
antenatal and family planning program care. However, during the delivery,
post-partum, and neonatal care  gaps were
found in which early breastfeeding initiation was not performed, the
post-partum care was not done for four visits, and the infant was given formula
milk respectively. Thus, health practitioners are expected to keep extending
the midwifery care in accordance with the standards recommended by the Ministry
of Health of the Republic of Indonesia.
