Pengaruh salbutamol terhadap kadar histamin serum sebelum pemberian atrakurium sebagai fasilitasi intubasi

Background : Atracurium is one of intermediate acting non depolarization neuromuscular blocking agents. Release of histamin is known to be the side effect of atracurium. Histamin itself is one of so many chemical mediators released by degranulated mast cells that is proceeded by IgE antibody interaction with antigen (alergen). Histamin can cause anaphylaxis reaction, both local or systemic. Salbutamol, a ß2 adrenergic agonist (sympathomimetic) and also a mast cell stabilizator, has been proved to prevent mast cell degranulation from which chemical mediators been released including histamin. Objective : To analize whether salbutamol has any influence on serum histamin level before administration of atracurium as fascilitating agent for intubation Methods : A clinical experimental study was conducted with design double blind randomized controlled trial. 30 samples of patient with physical status ASA I and ASA II, scheduled for operation under general anesthesia, were divided into 2 groups. Group A was treatment group, consisted of 15 patients receiving salbutamol 3x2 mg before operation (before administration of atracurium).Group B is control group, consisted of 15 patients receiving plasebo three times a day before surgery (before administration of atracurium). 3 cc of venous blood sample was carried away a moment before and 3 minutes after administration of atracurium. The samples were analized using quantitative histamine immunoassay ELISA reader machine. The serum histamin levels were calculated by statistic Independent Samples T Test. Result : Statistically, there is no significant difference of serum histamin level in treatment group (p = 0,635; p > 0,05). Conclusion : Oral salbutamol before administration of atracurium has a role on preventing histamin release. Keywords : Salbutamol, histamin, atracurium.
