Pengaruh Waktu Simpan Darah terhadap Kadar Laktat Dehidrogenase pada Packed Red Cells Tehnik Penyimpanan Darah Dengan Citrate Phosphate Dextrose – Saline Adenine Glucose Manitol

ABSTRAK
Latar belakang: Transfusi darah dilakukan dengan pemberian Whole Blood (WB) atau komponen darah sesuai kebutuhan. Laporan tahunan Unit Donor Darah (UDD) Palang Merah Indonesia (PMI) Kota Surakarta tahun 2014 Packed Red Cells (PRC) adalah 43.798 dari 68.211 (64.2%) permintaan darah transfusi. Peningkatan aktifitas serum LDH terjadi pada keadaan destruksi sel. Selama penyimpanan di bloodbank dengan suhu standar 2 – 6 0 C metabolisme glukosa dan hemolisis berkurangnya sehingga memperpanjang kelangsungan hidup PRC. Penyimpanan dengan CPD SAGM (Citrate, phosphate, dextrose, saline, adenine, glucose, manitol) memperpanjang penyimpanan PRC selama 42 hari dan mengurangi hemolisis..
Tujuan: Menganalisa pengaruh waktu simpan PRC dengan peningkatan kadar LDH.
Metode: Penelitian kuantitatif observasional dengan pendekatan crosssectional untuk membuktikan pengaruh waktu simpan terhadap peningkatan kadar LDH pada darah donor dengan CPD SAGM.. Pada 10 kantong PRC CPD SAGM pengukuran waktu simpan dilakukan pada hari ke 3, 7, 14, 21, 28, 35, 42, Pengukuran kadar LDH plasma darah dengan metode lactate /NAD referensi standar International Federation of Clinical Chemistry (IFCC).
Hasil: Hasil pengukuran kadar LDH dari 10 kantong darah menunjukkan pengaruh positif yang signifikan pada waktu simpan darah (R square = 94.8 %,
p < 0.01), sementara faktor lain tidak begitu berpengaruh (5.2 %).
Simpulan: Terdapat pengaruh positif yang signifikan waktu simpan darah terhadap kenaikan kadar LDH, semakin lama penyimpanan semakin tinggi kadar LDH yang menunjukkan proses hemolisis.

ABSTRACT
Background : Blood Transfusion are administered in form of whole blood (WB) or blood components necessary. Based on blood donation unit annual report of Indonesian Red Cross (IRC) Surakarta. Packed Red Cells (PRC) demand is 43,798 out of 68.211(64.2 %) blood transfusion demand. Lactate Dehydrogenase is intracellular enzyme, found in cells metabolism, its activity in serum increase in parallel with red cells destruction. Packed Red Cells storage in blood banks with standard temperature of 2-60 C slows down and reduces glucose metabolism and lysis to extend PRC life. The storage PRC with CPD SAGM ( Citrate, phosphate, dextrose, saline, adenine, glucose, and manitol) enables to prolong the storage (42 days) and reduce the level of hemolysis..
Objective : To analyse the effect of storage time of PRC the increase of LDH levels..
Methods : This quantitative observational and crosssectional study was conducted from June – July 2015. The storage time was divided by 3, 7, 14, 21, 28, 35, 42 days. The Lactate/ NAD method was performed to measure LDH level. ( Reffered to IFCC Standard). The statistical analysis was performed with linear regression, p < 0.05 was considered significant.
Result : The result of LDH level measurement from 10 blood bag revealed a significant positive effect of blood time storage ( R square = 94.8 % ; p < 0.01), while others factors didin’t really influence (5.2 %).
Conclusion : Since there is a significant positive effect of blood time storage, the longer the time storage the higher the LDH level which lead to hemolysis process.
