Keefektifan Mecta (Monitored Electro Convulsive Therapy With Anesthesia) pada Perbaikan Gejala Klinis dan Dampak terhadap Fungsi Kognitif pada Pasien Skizofrenia Sebuah Studi Kuantitatif di Unit Elektro Medis RSJD Surakarta

ABSTRAK
Latar belakang : Electro Convulsive Therapy baik secara konvensional maupun MECTA (Monitored Electro Convulsive Therapy with Anesthesia) masih menimbulkan kontroversi sampai saat ini, karena keefektifannya masih dipertanyakan, serta adanya ketakutan masyarakat yang dihubungkan dengan asumsi bahwa Electro Convulsive Therapy dapat menimbulkan gangguan fungsi kognitif. Dikarenakan banyaknya pro dan kontra mengenai efek terapeutik dan efek samping dari MECTA (Monitored Electro Convulsive Therapy with Anesthesia) dan kurangnya data yang valid di Indonesia, maka penelitian ini dilakukan dengan tujuan untuk mengetahui gambaran gejala klinis dan fungsi kognitif pada subjek Skizofrenia yang refrakter terhadap psikofarmaka yang mendapatkan MECTA, agar dapat memberikan penatalaksanaan yang cepat dan tepat dalam hal intervensi secara holistik dan komprehensif.
Tujuan : Untuk mengetahui perbaikan klinis dan fungsi kognitif pada subjek Skizofrenia yang mendapatkan MECTA (Monitored Electro Convulsive Therapy with Anesthesia).
Metode : Studi ini menggunakan metode penelitian kuantitatif berbentuk studi diskriptif analatik. Dalam penelitian ini tidak hanya mengamati subjek secara natural tetapi juga memberikan intervensi Monitored Electro Convulsive Therapy (MECTA). Pada penelitian ini dilakukan pengamatan perbaikan gejala klinis dan penurunan fungsi kognitif pada subjek Skizofrenia resistensi Clozapin pra dan pasca MECTA melalui penilaian dengan instrumen PANSS, MMSE dan CDT.
Hasil : Didapatkan perbaikan gejala klinis Skizofrenia: gejala positif, gejala negatif dan psikopatologi umum secara signifikan. Adanya penurunan fungsi kognitif yang nyata.
Kesimpulan : MECTA efektif memberikan perbaikan gejala klinis dalam pengobatan Skizofrenia dan menyebabkan penurunan fungsi kognitif.

ABSTRACT
Background : Electroconvulsive Therapy either conventionally or MECTA (Electro Convulsive Therapy with Monitored Anesthesia) is still causing controversy currently, because its effectiveness is still questionable, as well as the fear in the public associated with the assumption that the Electroconvulsive Therapy can lead to impaired cognitive function. Due to the many pro and contra regarding the therapeutic effects and side effects of MECTA (Monitored Electro convulsive Therapy with Anesthesia) and the lack of valid data in Indonesia, this study was conducted in order to describe the clinical symptoms and cognitive function in subjects schizophrenia refractory to psychotic drugs getting MECTA, in order to provide fast and precise management in a holistic and comprehensive interventions.
Objective : To describe the clinical symptoms and cognitive function in schizophrenia subjects who obtained MECTA (Electro convulsive Therapy with Monitored Anesthesia).
Methods : This study uses a quantitative research methods analytic descriptive cross sectional studies. In this study not only observe the subject naturally, but also provide intervention with Monitored Electro convulsive therapy (MECTA). This study was observed improvement of clinical symptoms and cognitive decline in subjects with schizophrenia clozapine resistance pre- and post MECTA through assessment with instruments such as PANSS, MMSE and CDT.
Results : Obtained improvement of clinical symptoms of schizophrenia: positive symptoms, negative symptoms and general psychopathology symptoms significantly. A real decline in cognitive function was found.
Conclusion : MECTA is effective in improvement of clinical symptoms in the treatment of schizophrenia clozapine resistance and cause a decrease in cognitive function.
