Pengaruh Kalsitriol terhadap Vep1% dan Kadar IL-5 Plasma pada Asma

ABSTRAK
Latar Belakang: Salah satu tujuan jangka panjang penatalaksanaan Asma untuk mencapai kontrol gejala yang baik dan meminimalkan risiko eksaserbasi di masa mendatang. Pendekatan pencapaian kontrol Asma dilakukan melalui terapi medikamentosa yaitu pengobatan pengendali (controller), pelega (reliever), dan terapi suplementasi yaitu vitamin D. Vitamin D yang digunakan adalah kalsitriol (1,25[OH]2D3), bentukan aktif vitamin D. Peran suplementasi vitamin D pada Asma melalui beberapa cara, yakni imunomodulator, antiviral dan antimikroba, meningkatkan respons steroid, dan menurunkan atopi. Beberapa penelitian pengaruh kalsitriol telah dilakukan dengan hasil penelitian yang berbeda. Biomarker dalam penegakkan diagnosis asma secara tepat dan cepat telah banyak dikenal, diantaranya adalah IL-5. Penelitian ini bertujuan menganalisis pengaruh pemberian vitamin D terhadap VEP1% dan kadar IL-5 plasma pada penderita Asma tidak dalam eksaserbasi.
Metode: jenis penelitian uji klinis eksperimental dengan rancangan group pretest - posttest. Sampel penelitian adalah penderita Asma yang tidak dalam eksaserbasi di RSUD Dr. Moewardi, Surakarta mulai tanggal 25 September 2015 hingga 31 Oktober 2015. Pengambilan sampel dengan cara purposive sampling. Besar sampel 32 pasien dibagi menjadi 16 pasien sebagai kelompok perlakuan (diberikan kalsitriol) dan 16 pasien sebagai kelompok kontrol (diterapi Asma).
Hasil: pemberian kalsitriol meningkatkan nilai VEP1% namun tidak berpengaruh secara signifikan (78,08?6,35 vs 77,80?5,74; p=0,89) dan secara signifikan menurunkan kadar IL-5 plasma (0,18?0,15 vs 0,62?0,56; p=0,002).
Simpulan: pemberian kalsitriol pada pasien asma dapat meningkatkan nilai VEP1%, meskipun tidak signifikan, serta menurunkan kadar IL-5 plasma.

ABSTRACT
Background: one long term goal of Asthma management to achieve good controled and minimized the risk of exacerbation in the next future. Approached the achievement of Asthma controled had done through medicamentosa included controller and reliever theraphy, supplementation theraphy such as vitamin D in the active form, calcitriol (1,25[OH]2D3), the role of calcitriol supplementation in Asthma due to several ways, included immunomodulator, antiviral and antimicrobial, enhancing steroid respons, and lowering atophy. There had many studied about effects of calcitriol with differences result. Biomarker in the diagnosis of Asthma accurately and quickly had been widely known, such as interleukin (IL)-5.
Objective: this study aimed to determine the effects of vitamin D supplementation in VEP1% and plasma level of IL-5 in Asthma without exacerbation.
Methode: this study had conducted through clinical trial experimental group pretest posttest designed, sample were Asthma patients without exacerbation in DR Moewardi Hospital Surakarta from 25 September until 31 October 2015. Sampling by purposive sampling, total 32 patients devided into 16 patients in treatment group (given calcitriol), 16 patients in control group given placebo.
Result: the results showed oral administration of calcitriol 2x0,25 mcg compared to placebo not significantly increased VEP1% (78,08?6,35 vs 77,80?5,74; p=0,898), but significantly decreased IL-5 plasma level (0,18?0,15 vs 0,62?0,56; p=0,002).
Conclusion: administration of calcitriol could increased level of VEP1% although not significant, and decreased plasma level of IL-5 in Asthma patient
