Clean and Healthy Life Behavior (PHBS) is all health behavior that is carried out on awareness, so that family members can help themselves in the health sector and play an active role in public health. However, in their behavior, the community has not been able to know the state of the environment they live in because there is no information about PHBS in a community environment. Based on these problems, it is necessary to create an information system that can assist the Monitoring of Clean and Healthy Life Behavior (PHBS) in an environment which is expected to assist in monitoring health in an environment. According to Husein and Wibowo (2006), an information system is a set of interrelated components that function to collect, store, and distribute information to support decision-making and oversight in organizations. The application development stage includes analysis, system design, implementation and testing. Where the design is implemented into a Clean and Healthy Living Information System using the PHP programming language, Laravel framework, MySQL database, and this testing system uses black box text testing. From the problems above, it is necessary to make a monitoring application, where later the application can be very useful for unifying the state of health of an environment. Likewise, people in an area can know the conditions and circumstances of their environment.
