HUBUNGAN KADAR HBA1C DENGAN KECEPATAN HANTAR SARAF NERVUS PERONEAL PADA PASIEN POLINEUROPATI DIABETIK

Latar Belakang: Diabetes melitus tipe 2 (DMT2) berisiko menyebabkan neuropati diabetik, termasuk neuropati peroneal yang berdampak pada fungsi sensorimotor. Kecepatan hantar saraf (KHS) digunakan untuk menilai kerusakan saraf, sementara kadar HbA1c mencerminkan kontrol glikemik. Penelitian ini bertujuan menganalisis hubungan HbA1c dengan KHS nervus peroneal pada pasien DMT2 di RSUD Dr. Moewardi Surakarta untuk mendukung deteksi dini dan pencegahan neuropati.

Metode: Metode penelitian ini menggunakan desain cross-sectional. Penelitian dilakukan di RSUD Dr. Moewardi Surakarta pada September–Desember 2024. Sampel terdiri dari 52 pasien polineuropati diabetik yang dipilih secara purposive sampling. Variabel bebas adalah kadar HbA1c, variabel terikat adalah kecepatan hantar saraf (KHS) nervus peroneal, sementara variabel perancu meliputi usia, riwayat merokok, hipertensi, dan lama menderita DM. Analisis hubungan antara kadar HbA1c dan KHS dilakukan menggunakan uji korelasi Pearson atau Spearman dengan bantuan SPSS.

Hasil: Penelitian melibatkan 52 pasien dimana proporsi wanita (46,2%) dan pria (53,8%). Pada uji spearman rank didaptkan nilai p<0>r= -0,729 dan r= -0,791 menunjukkan korelasi signifikan dengan arah negatif.

Simpulan: Terdapat hubungan yang signifikan secara statistik antara kadar HbA1c dan kecepatan hantar saraf (KHS) nervus peroneal pada pasien dengan polineuropati diabetik di RSUD Dr. Moewardi Surakarta.

ABSTRACT

THE RELATIONSHIP BETWEEN HBA1C LEVELS AND PERONEAL NERVE CONDUCTION VELOCITY IN DIABETIC POLYNEUROPATHY PATIENTS

Background: Type 2 diabetes mellitus (T2DM) carries the risk of diabetic neuropathy, including peroneal neuropathy that impacts sensorimotor function. Nerve conduction velocity (NCV) is used to assess nerve damage, while HbA1c levels reflect glycaemic control. This study aims to analyse the relationship between HbA1c and peroneal nerve’s NCV in T2DM patients at Dr Moewardi Hospital Surakarta to support early detection and prevention of neuropathy.

Methods: This research method used a cross-sectional design. The study was conducted at Dr Moewardi Hospital Surakarta in September-December 2024. The sample consisted of 52 diabetic polyneuropathy patients selected by purposive sampling. The independent variable was HbA1c level, the dependent variable was peroneal nerve conduction velocity (NCV), while the confounding variables included age, smoking history, hypertension, and duration of DM. The relationship between HbA1c and NCV was analysed using Pearson or Spearman correlation test with the use of SPSS.

Results: The study included 52 patients, with 46.2?male and 53.8% male. The Spearman rank test showed a significant relationship between HbA1c levels and diabetes duration with peroneal nerve’s NCV (p < 0 xss=removed xss=removed>

Conclusion: There is a statistically significant relationship between HbA1c levels and peroneal nerve conduction velocity in diabetic polyneuropathy patients at RSUD Dr. Moewardi Surakarta.
