Roro Dyah Sekar Djatiningrum, G0021192, 2025. Hubungan Skor Child-Pugh Dengan Derajat Varises Esofagus Pada Pasien Sirosis Hepatis di RSUD Dr. Moewardi.

Pendahuluan: Hasil rekam medis pasien di RSUD Dr. Moewardi  menunjukkan sebanyak  5.943 pasien dengan diagnosis sirosis hepatis pada periode Januari 2023 – Januari 2024. Sebanyak 1.178 pasien dengan sirosis hepatis menjalani rawat inap dan 4.765 pasien sirosis hepatis menjalani rawat jalan. Sebanyak 11,48% pasien sirosis hepatis juga terdiagnosis dengan varises esofagus. Varises esofagus (VE) merupakan komplikasi dari sirosis hepatitis akibat perdarahan yang berulang. VE terjadi karena adanya pelebaran vena esofagus distal submukosa yang menghubungkan sirkulasi portal dan sistemik sehingga terjadi pecahnya varises.

Metode Penelitian dilakukan dengan metode observational analytic dengan pendekatan cross-sectional pada 68 pasien. Variabel yang diteliti meliputi usia, jenis kelamin, prothrombin time, bilirubin, albumin, asites, ensefalopati hepatik, dan skor child-pugh.

Hasil dan Pembahasan :  Hasil penelitian menunjukkan bahwa  mayoritas pasien dengan varises esofagus berada pada rentang usia 40-60 tahun dengan jenis kelamin laki-laki. Sebagian besar pasien memiliki varises esofagus derajat III (42,6%). Berdasarkan analisis bivariat didapatkan skor child pugh berhubungan dengan varises esofagus. Analisis multivariat didapatkan bahwa prothrombin time, bilirubin, albumin, asites, dan ensefalopati hepatik berhubungan dengan derajat varises esofagus.

Kesimpulan : Kesimpulan yang didapat yaitu prothrombin time, bilirubin, albumin, ensefalopati hepatik, dan skor child-pugh berhubungan dengan derajat varises esofagus. Asites tidak memiliki hubungan spesifik dengan derajat varises esofagus.

Introduction: Patient’s medical records at RSUD Dr. Moewardi showed that there are 5,943 patients diagnosed with liver cirrhosis in the period January 2023 – January 2024. As of 1,178 patients with liver cirrhosis underwent inpatient treatment and 4,765 patients with liver cirrhosis underwent outpatient treatment. 11.48% of liver cirrhosis patients were also diagnosed with esophageal varices. Esophageal varices (VE) is a complication of cirrhosis due to recurrent bleeding. VE occurs due to dilation of the distal submucosal esophageal veins that connect the portal and systemic circulation, resulting in rupture of varices.

Methods : This research used an observational analytic method with a cross-sectional approach on 68 patients. Variables studied included age, gender, prothrombin time, bilirubin, albumin, ascites, hepatic encephalopathy, and child-pugh score.

Results : Results of this research show that the majority of patients with esophageal varices are in the age range of 40-60 years and male. Most patients have grade III esophageal varices (42.6%). Based on bivariate anaylisis, child-pugh score related to the stage of esophageal varices. Based on multivariate analysis, prothrombin time, bilirubin, albumin, ascites, hepatic encephalopathy score  related to the stage of esophageal varices.

Conclusion: In conclusion, prothrombin time, bilirubin, albumin, hepatic encephalopathy, and child pugh score are related to the stage of esophageal varices. Ascites does not have a spesific relation with esophageal varices stage.
