× Abstrak Latar Belakang. Angka kematian akibat sepsis masih cukup tinggi baik di negara maju maupun negara berkembang. Penggunaan kortikosteroid dosis rendah pada sepsis bermanfaat dalam memperbaiki fungsi adrenal pada keadaan stres dan mengurangi komponen anti-inflamasi. Skor APACHE II apabila dikombinasikan dengan gambaran penyakit yang jelas berguna untuk stratifikasi penyakit akut berdasarkan prognosis dan dapat membantu peneliti membandingkan keberhasilan bentuk terapi baru. Tujuan Penelitian. Penelitian ini bertujuan memberikan penilaian klinis efek pemberian kortikosteroid dosis rendah terhadap perbaikan klinis pasien sepsis dengan menggunakan evaluasi skor APACHE II. Metodologi. Jenis penelitian ini adalah eksperimen dengan randomisasi. Penelitian dilakukan di bangsal Penyakit Dalam, bagian Ilmu Penyakit Dalam RSUD Dr. Moewardi Surakarta, mulai bulan Desember 2008 sampai Mei 2009. Pada kelompok perlakuan mendapatkan terapi suplementasi tambahan berupa metilprednisolon intravena 20mg / 8 jam. Dilakukan evaluasi skor APACHE II pada saat datang, hari ke-3, ke-5 dan ke-7. Uji hipotesis dengan menggunakan uji t tidak berpasangan atau uji Mann Whitney untuk alternatifnya. Hasil. Didapatkan 26 subyek penelitian yang secara lengkap diikuti selama 7 hari. Pada awal penelitian didapatkan kondisi yang setara antara kelompok perlakuan dan kontrol. Tidak didapatkan perbedaan yang secara statistik bermakna untuk skor APACHE II dan PDR hari ke-3 dan ke-5 (p >0,05). Didapatkan perbedaan yang secara statistik bermakna pada skor APACHE II (p =0,017) dan PDR (p =0,017) hari ke-7. Kesimpulan. Pemberian kortikosteroid dosis rendah pada terapi sepsis dapat memperbaiki keadaan klinis pasien sepsis dengan menggunakan evaluasi skor APACHE II. Kata kunci : Sepsis-Kortikosteroid dosis rendah-Skor APACHE II Abstract Background. Mortality rate in Sepsis still remains higher both in developed and developing countries. Low dose corticosteroid has beneficial effect to ameliorate adrenal function due to stress condition and lessen anti inflammation components. APACHE II score is one of the most predictor factors for grading and evaluate the severity. If APACHE II score combined with the sign of the disease is useful to grade acute disease based on prognosis and assist researcher to compare prospering of new therapy. Research Objectives. Objective of this study is offering clinical evaluation the effect of low dose corticosteroid for septic patient using APACHE II score. Method. Type of this study is RCT (randomize clinical trial). The study is conducted in Department Internal Medicine-RSUD Dr Moewardi, Surakarta, December 2008 – May 2009. In the subjects with corticosteroid treatment obtain metylprednisolon I.V 20 mg/8 hours. The evaluation using APACHE II score on the admission day, third , fifth and the seventh day. Hypothesis test is using unpair t-test or mann whitney test for alternative. Results. There were 26 subjects with fully follow-up evaluation for 7 days. At the initial study were obtained the same condition between subject with corticosteroid treatment and without corticosteroid. There is no statistically significant for APACHE II score and PDR in the third and fifth day of follow-up (p >0,05) but there is significant result for APACHE II score (p =0,017) and PDR (p =0,017) in the seventh day of follow-up. Conclusion. Low dose corticosteroid can ameliorate clinical condition of the septic patients using APACHE II score. Keywords : Sepsis-Low dose corticosteroid-APACHE II score
