Latar Belakang: Masih rendahnya perilaku pemberantasan sarang nyamuk DBD dipengaruhi oleh beberapa faktor, antara lain: “faktor kepercayaan, nilai, sikap, usia”. Dalam membentuk perilaku atau tindakan yang positif dapat dibentuk melalui suatu proses dan berlangsung dalam interaksi manusia dan lingkungan. Faktor yang mempengaruhi perilaku adalah pengetahuan, persepsi, emosi, motivasi dan lainnya.
Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara pengetahuan dan motivasi dengan perilaku kepala keluarga dalam pemberantasan sarang nyamuk demam berdarah dengue di wilayah kerja Puskesmas Loa Ipuh Kutai Kartanegara Kalimantan Timur.
Metode: Penelitian ini merupakan penelitian cross sectional study dengan analisis deskriptif korelasional dan teknik sampel menggunakan proportional sampling. Jumlah sampel sebanyak sebanyak 113 orang.
Hasil: Hasil penelitian menyimpulkan bahwa: 1) pengetahuan mempunyai hubungan yang signifikan terhadap perilaku kepala keluarga dalam pemberantasan sarang nyamuk demam berdarah dengan nilai t = 5,282 < t 5%; 113 = 1,66 dan p = 0,000 < 0,05; 2) motivasi mempunyai hubungan yang signifikan terhadap perilaku kepala keluarga dalam pemberantasan sarang nyamuk demam berdarah dengan nilai t = 3,792 < t 5%; 113 = 1,66 dan p = 0,000 < 0,05; dan 3) secara simultan pengetahuan dan motivasi berpengaruh terhadap perilaku kepala keluarga dalam pemberantasan sarang nyamuk demam berdarah dengan nilai R square sebesar 0,701, artinya masih ada faktor yang berpengaruh diluar pengetahuan dan motivasi sebesar 1- 0,701 = 0,299 = 29,9%. Sedangkan model prediktifnya dalam bentuk linear regresi sebagai berikut : y = 0,917 + 0,634 X1 + 0,099 X2 dengan F hitung = 25,141 > F 5%;2;110 = 3,92 dan p = 0,000 < 0,05.
Kesimpulan: Pengetahuan dan motivasi berpengaruh terhadap perilaku kepala keluarga dalam pemberantasan sarang nyamuk demam berdarah dengue di Wilayah Kerja Puskesmas Loa Ipuh Kabupaten Kutai Kartanegara

ABSTRACT
Sri Suharti R., S540809420. The Correlation of the Knowledge and the Motivation to the Behavior of the Household Heads on the Eradication of Mosquito Nest (at the Working Area of the Community Health Center of Loa Ipuh, Kutai Kartanegara Regency).
Background: The low eradication behavior of dengue mosquito breeding is influenced by several factors, among others: "trust factor, values, attitudes, age." In shaping positive behaviors or actions that can be formed through a process and takes place in human interaction and the environment. Factors that influence the behavior is knowledge, perception, emotion, motivation and others.
Objective: The objective of the research is to investigate the correlation of the knowledge and the motivation to the behavior of the household heads on the eradication of mosquito nest at the working area of the Community Health Center of Loa Ipuh, Kutai Kartanegara regency, Kalimantan Timur province.
Research Method: The research used a descriptive correlational method with a cross sectional research design. Its samples consisted of 113 household heads and were taken by using a proportional random sampling technique.
Result: The results of the research are as follows: 1) there is a significant correlation between the knowledge and the behavior of the household heads on the eradication of the mosquito nest as indicated by the value of t = 5.282 < t 5%; 113 = 1.66 and p = 0,000 < 0.05; 2) there is a significant correlation between the motivation and the behavior of the household heads on the eradication of the mosquito nest as shown by the value of t = 3.792 < t 5%; 113 = 1.66 and p = 0,000 < 0.05; and 3) there is a simultaneously significant correlation of the knowledge and the motivation to the behavior of the household heads on the eradication of the mosquito nest as pointed out by the value of R square = 0.701, meaning that there are still influencing factors other than the knowledge and the motivation amounting to 1- 0.701 = 0.299 = 29,9%. Its predictive model with the equation of linear reggression is y = 0,917 + 0,634 X1 + 0,099X2 with the value of F count = 25,141 > F 5%;2;110 = 3.92 and p = 0.000 < 0.05.
Conclusion. Knowledge and motivation influence the behavior of household heads in the eradication of dengue mosquito breeding in the Community Health Center of Loa Ipuh, Kutai Kartanegara regency.
