×This final project report was written based on the job training I have done in PKU Muhammadiyah Hospital of Surakarta, starting on March 10th, 2014 until April 11th, 2014. The objectives of this final project were to describe the questionnaire used in the hospital, to know the effectiveness of the questionnaire as communication channel, and to propose some improvements to the use of the questionnaire. During the job training, I learned a few things related to the distribution of the questionnaire i.e. the questionnaire design, the objective of the questionnaire, the informants selected, the functions of the questionnaire, and the evaluation of the questionnaire.
Questionnaire is one of communication channels in PKU Muhammadiyah Hospital of Surakarta. Questionnaire is used by two divisions, which are Public Relations division and Marketing division. The questionnaire is used to know the customer satisfaction and to carry out direct communication with patients. However, the use of the questionnaire as communication channel is less effective because not all objectives are achieved. Some problems occured in distributing questionnaire. Based on the existing condition, I propose some improvements to the use of the questionnaire to be more effective, which are making the appearance of the questionnaire more attractive to attract the respondents in filling out the questionnaire, providing free gift to reduce the lack of response, determining the standard number of questionnaire, implementing better time management in distribution, and increasing the number of officer.
Hopefully this final project report can be used as consideration for Public Relations division in maintaining and improving the effectiveness of the questionnaire and it can be beneficial to the readers especially those who are interested in Public Relations.
