ABSTRAK.
Latar Belakang : Latar belakang dilakukannya penelitian ini adalah Kondisi distress berkaitan dengan konsentrasi kortisol darah dan konsentrasi IL–6 plasma maupun distribusinya di susunan saraf pusat. Pajanan stresor merangsang hipotalamus, pituitari dan kelenjar adrenal, yang membentuk axis HPA serta terkait dengan dampak stres, seperti peningkatan kadar kortisol dan katekolamin yang berhubungan erat dengan kadar serotonin dan dopamin dalam otak. IL-6 merupakan salah satu sitokin pro inflamasi yang terekspresi oleh kortisol. Alprazolam diberikan secara oral dapat mengurangi kecemasan pada pasien yang akan menurunkan kadar IL-6 plasma.
Tujuan : Mengamati apakah ada pengaruh pemberian Alprazolam 0,5 mg dengan tanpa pemberian Alprazolam terhadap kadar Il-6 serum pasien perioperatif.
Metode :Merupakan penelitian eksperimental laboratorik dengan desain randomized control trial.30 pasien dibagi menjadi dua kelompok.K1 merupakan kelompok kontrol yang terdiri dari 15 pasien dan K2 merupakan kelompok eksperimen terdiri dari 15 pasien yang diberikan Alprazolam peroral 0.5 mg ,malam sebelum operasi. Pemeriksaan kadar IL-6 dilakukan dengan menggunakan metode Elisa.Kit yang digunakan adalah Human IL-6 Immunoassay, Quantikine Elisa dari R&D Systems pasca operasi. Dianggap memiliki kemaknaan statistik apabila nilai p yang diperoleh adalah p ≤ 0,05.
Hasil : Ada perbedaan selisih kadar IL-6 serum yang signifikan antara kelompok kontrol dan perlakuan. Rata-ratakontrol pada pre kadar IL-6 1,07±0,18 pg dan post 1,34±0,30 pg (p=0,06). Sedangkan rata-rata kadar IL-6 perlakuan pada pre 2,42±0,34 pg dan post 1,57±0,49 pg (p=0,01). Data menunjukkan selisih kadar IL-6 serum kelompok perlakuan lebih rendah dibandingkan kelompok kontrol t = -5,432 (p = 0,001).
Kesimpulan : Pemberian premedikasi alprazolam menurunkan kadar IL-6 serum pasien perioperatif yang secara statistik bermakna.

ABSTRACT
Background : The background of this research was the distress condition associated with blood cortisol level and IL-6 plasma level and also their distribution to the central nervous system. Stressor exposure stimulates hypothalamus, pituitary and adrenal gland which makes the HPA axis, it is associated stress impact, like the increase of cortisol and catecolamine which closely related with serotonin level and dopamine inside the brain. IL-6 is one of the pro inflammation cytokines which is expressed by cortisol. Alprazolam which is given orally can reduce anxiety in patient who is about to decrease the IL-6 plasma level.
Objective : To monitor whether there is an effect in administration of Alprazolam 0.5 mg or without the administration to the IL-6 serum level of perioperative patient.
Methods : This research is laboratory experimental with randomized control trial design. 30 patients were divided into two groups. Group 1 (K1) was a control group of 15 patients and group 2 (K2) was an experimental group of 15 patients who each received Alprazolam 0.5 mg orally, a night before operation. The IL-6 level examination was done by Elisa Kit method which was using Human IL-6 Immunoassay, Quantikine Elisa from R&D Systems after operation. It was considered to have statistical value if p value is p ≤ 0.05.
Result : There was significant difference in IL-6 serum level between control and experimental groups. The average IL-6 level control in pre was IL-6 1.07±0.18 pg and post was 1.34±0.30 pg (p=0.06). While the average IL-6 level in pre was 2.42±0.34 pg and post was 1.57±0.49 pg with (p=0.01). The data shows difference of IL-6 serum level of experimental group was lower than control group t= -5.432 (p=0.001).
Conclusion: Alprazolam Premedication reduces IL-6 serum level in perioperative patient is statistically significant value.
