×ABSTRAKLatar Belakang : Otitis media Supuratif kronik (OMSK) merupakan penyakitTHT terbanyak di negara berkembang. Tulang secara anatomis merupakan sistempertahanan jika terjadi destruksi tulang akibat proses infeksi menyebabkanperluasan dan komplikasi sehingga menjadi masalah kesehatan. Pada rematoidartritis (RA) dan periodontitis, IL-17 memberi kontribusi terjadi destruksi tulang.IL-17 menginduksi RANKL, matriksmetaloproteinase (MMPS), nitric oxide(NO) dan prostaglandine (PGE2) sebagai faktor penyebab destruksi tulang. IL-17meningkatkan regulasi (up-regulation) sitokin: IL-1, TNFα dan IL-6.Bahan dan Cara : Rancangan penelitian observasional analitik potong lintang.Sampel terdiri dari 31 sampel. Penilaian tingkat destruksi tulang berdasarkan Ct-Scan fokus mastoid dan atau temuan operasi mastoidektomi secara mikroskopis.Tiap sampel dilakukan pemeriksaan IL-17 secara immunohistokimia. Dilakukanperhitungan intensitas distribusi skor. Analisis data menggunakan uji korelasi.Hasil : Pada pasien dengan Ekspresi IL-17 negatif didapatkan: 4 pasien (12,9%)tanpa destruksi; 6 pasien (19,4%) dengan destruksi tingkat 1; 6 pasien (19,4%)dengan destruksi tingkat 2 dan 1 pasien (3,2%) dengan destruksi tingkat 3. Padapasien dengan ekspresi IL-17 positif lemah didapatkan: 12 pasien (38,7%) dengandestruksi tingkat 2 dan 1 pasien (3,2%) dengan destruksi tingkat 3. Pada pasiendengan ekspresi IL-17 positif sedang didapatkan: 1 (3,2%) pasien dengandestruksi tingkat 3. Koefisien korelasi (Rs) antara ekspresi IL-17 dengan tingkatdestruksi sebesar 0,595 berdasarkan uji Spearman’s Rank dengan signifikansi (p) :0,001 (p < 0,05).Kesimpulan : Terdapat hubungan antara ekspresi IL-17 di jaringan granulasikavum timpani dengan tingkat destruksi tulang pada pasien OMSK.Kata Kunci : Interleukine 17, OMSK, Destruksi Tulang, osteoklasgenesisABSTRACTBackground : Chronic Supurative Otitis Media (CSOM) is the most ENTdiseases in developing countries. Bone is the defense anatomically, therefore incase of bone destruction due to a chronic infection process lead to the expansionand complication that cause health problems. In the rheumatoid arthritis (RA) andperiodontitis reported that IL-17 contributes occurrence of bone destruction. IL-17induces RANKL, matrixmetalloprotheinase (MMPs), nitric oxide (NO) andprostaglandine (PGE2) as a factor of bone destruction. IL-17 can increaseregulation (up-regulation) of cytokines such as IL-1, TNFα and IL-6.Material and Methods : The study was observational analytic with crosssectional. The samples consisted of 31 samples. Assessment of bone destructionjudged from the Ct-Scan focused on the mastoid findings and or microscopicfindings on mastoidectomy operation. Each sample was examined IL 17 throughimmunohystochemical techniques. Calculation of the intensity distribution ofscores was conducted. Data analysis using correlation test.Result : The expression IL-17 had negative consisted of 4 patient (12,9%)without bone destruction; 6 patient (19,4%) with first degree of bone destruction;6 patient (19,4%) with second degree and 1 patient (3,2%) with third degree. Theexpression IL-17 had weak consisted of 12 patient (38,7%) with second degree ofbone destruction and 1 patient (3,2%) patient with third degree. The expressionIL-17 had mild consisted of 1 patient (3,2%) with third degree of bonedestruction. CSOM had a coefficient correlation (Rs) 0,595 with Spearman’sRank test. Statistically had significant differences of interleukin 17 expressionwith degree of bone destruction in CSOM (p) value 0,001 (p < 0,05).Conclution : There is a correlation between the expression of IL-17 in cavumtymphani granulation with the degree of bone destruction in CSOM.Keywords : Interleukine 17, Chronic Otitis Media, bone destruction, Osteoclastgenesis
