ABSTRAK
Gangguan resorpsi dan destruksi tulang dapat terjadi pada OMSK maligna
yang ditandai dengan pembentukan kolesteatoma dan dengan jaringan granulasi.
RANKL memainkan peran penting dalam osteoklastogenesis. Interaksi antara
receptor activator of nuclear factor ?B (RANK) dan RANKL adalah tanda awal
osteklastogenesis dan aktivasi osteoklas. OPG merupakan antagonis reseptor
endogen dan setelah berikatan dengan RANKL menghambat osteoklastogenesis,
sehingga menghambat proses resorpsi tulang. Penelitian ini bertujuan mengetahui
hubungan rasio ekspresi RANKL dan OPG dengan destruksi tulang pada pasien
OMSK.
Penelitian observasional analitik dengan desain cross sectional. Sebanyak 22
sampel OMSK diperiksa jaringan granulasi telinga tengah menggunakan
pemeriksaan imunohistokimia dengan perhitungan IDS. Tingkat destruksi tulang
dibagi menjadi tingkat 0: tidak terdapat destruksi, tingkat 1: destruksi skutum dan
osikel, tingkat 2: destruksi tegmen dan seluruh osikel, tingkat 3: destruksi tegmen,
seluruh osikel, kanalis fasialis, dinding liang telinga, sinus sigmoid, koklea dan
dinding tulang mastoid. Teknik korelasi yang digunakan spearman’s rank.
Perhitungan rata-rata rasio RANKL/OPG pada pasien dengan destruksi
tulang tingkat 0 = 1,43 ??0,72, pasien dengan destruksi tulang tingkat 1 = 4,10 ?
2,58, pasien dengan destruksi tulang tingkat 2 = 6,17 ± 4,39 dan pasien dengan
destruksi tulang tingkat 3 = 5,64 ± 3,11 (r=0,646). Semakin tinggi rasio
RANKL/OPG semakin tinggi pula derajat destruksi tulang. Terdapat hubungan
yang bermakna (p=0,001) antara rasio ekspresi RANKL/OPG dengan tingkat
destruksi tulang. Rasio ekspresi RANKL/OPG sebagai salah satu faktor prediksi
prognosis pada OMSK.

ABSTRACT
The complication of CSOM with granulation tissue and cholesteatoma
begins with impaired bone resorption and destruction. Bone destruction always
happens in malignant type of CSOM. Receptor Activator of Nuclear Factor ?B
Ligand (RANKL) and Osteoprotegerin (OPG) are a key factor that controls
osteoclastogenesis. Osteoclastogenesis and osteoclast activation are initiated by
interaction between RANK and RANKL. OPG is an endogenous receptor that
competes with RANK to interact with RANKL, inducing osteoblastogenesis and
inhibiting bone resorpsion. The aim of the study was determine the correlation
between ratio Receptor Activator of Nuclear Factor ?B Ligand and
Osteoptotegerin and Bone Destruction in Chronic Suppurative Otitis Media
This study was an analytic observational study with cross sectional design.
A total of 22 specimens of granulations tissues the arrived from were examined by
using immunohistochemical assay. Bone destructions were classificatied by four
level (0: there is no destruction, level 1: destruction the scutum and ossicles, level
2: destruction the tegmen and all the ossicles, level 3: destruction the tegmen, all
the ossicles, the canal facial, the walls of ear canal, the sinus sigmoid, the cochlear
and the mastoid bone wall). All generated data were analyzed by using statistical
test.
The higest of RANKL/ OPG ratio was observed in patient with bone
destruction level 2 (6.17 ± 4.39). Mean while average of RANKL / OPG ratio in
patient with other bone destruction level was lower than average that of in patient
with bone destruction level 2. There was a significant correlation (p = 0.001)
between the expression of RANKL / OPG ratio and bone destruction. The
expresion ratio of RANKL/OPG is one of the prognosis prediction fact.
