ABSTRACK
Scope: Indonesia has a high mortality rate annually, particularly the maternal and neonatal mortality rate. One of the efforts to improve the quality of maternal and neonatal health services is implementation of integrated midwifery care or continuity of care (COC) which is in line with midwives’ competencies.

Implementation: The COC was extended to Mrs. A with the frequencies of two vistis of antenatal care, one visit of delivery care, four visits of postpartum care, three visits of neonatal care, and two visits of family planning program care.
Evaluation: The COC on Mrs. A during her gestational and postpartum periods went on normally without complications. During the delivery care, Mrs. A was found to suffer from myopia with 3 diopters, but it was still in the physiological category, and Mrs. A could gave a birth at Community Health Center according to an ophthalmologist. In the neonatal care, her infant did not have any abnormalities and problems. Mrs. A chose lactational amenorrheaVmethod for her family planning program. Conclusion and Recommendation: No gap was found during the implementation of the COC from antenatal to family planning program periods.
