D III Midwifery Study Program, Vocational School, Sebelas Maret University, 2026. Scope: Continuity of Care (COC) is a comprehensive care from pregnancy, childbirth, postpartum, newborns to family planning according to midwifery service standards. Implementation: Care for Mrs. R began at 34 +3 weeks of pregnancy. During pregnancy, a history of a cesarean section operation less than two years ago was found, the KSPR score indicated a very high-risk pregnancy category, and delivery was by cesarean section. Postpartum care was carried out through four visits focusing on monitoring post-cesarean wound healing, proper breastfeeding techniques, and EPDS screening at the last visit. Newborn care included three neonatal visits to monitor the management of physiological jaundice adaptation. Family planning services were provided through counseling, with the mother choosing the condom method. Evaluation: Midwifery care was implemented according to standards, with the mother and baby in good condition. Delivery was by cesarean section, so monitoring was carried out in the fourth stage after the mother was transferred to the ward. Conclusions and Recommendations: Comprehensive continuity of midwifery care was implemented. Improvement of midwifery services is needed to support early detection of complications and improve maternal and infant health.
