ABSTRACT Tambaksari District is one of the endemic areas for Dengue Hemorrhagic Fever (DHF) in Surabaya. High population density, poor sanitation, and low public awareness of environmental cleanliness are major factors in the spread of DHF. The use of chemical larvicides, such as abate, has led to resistance in Aedes aegypti mosquitoes, necessitating an alternative, environmentally friendly, and sustainable approach. A potential solution is the utilization of lemongrass leaves in water storage containers, as they contain natural larvicidal compounds to control mosquito populations. This study aims to: (1) assess environmental conditions and community knowledge, attitudes, and practices regarding DHF prevention; (2) analyze the implementation of DHF prevention programs in Pacar Keling Village, Tambaksari District, Surabaya (3) evaluate the characteristics and health promotion model needed to enhance community awareness and preventive actions; (4) determine the effect of the health promotion model on improving public knowledge, attitudes, and practices about DHF prevention through the use of lemongrass leaves in water storage containers. This study employs a mixed-methods approach with an exploratory sequential design. The case study was conducted in Pacar Keling Village, Tambaksari District, Surabaya, involving 132 community health volunteers across 12 neighborhood units (RW). Additionally, seven key informants were interviewed, including representatives from the local health center (Puskesmas), village administration, health department, mosquito surveillance officers, community movement for mosquito eradication, local leaders, and residents. Qualitative data were analyzed using Miles and Huberman’s interactive model, while quantitative data were tested using comparative analysis. The findings indicate that (1) physical environmental factors, such as land area, temperature, humidity, and rainfall, directly impact mosquito breeding, while social factors, including communal living habits and interaction patterns, influence DHF transmission; (2) the DHF prevention program in Pacar Keling Village has effectively reduced disease incidence and controlled its spread through mosquito eradication campaigns, abate promotion, and public education. Collaboration between government agencies, health institutions, and the local community has enhanced vector control outcomes. Increased public awareness of DHF risks has motivated better preventive behaviors, while mosquito control measures have reduced Aedes aegypti populations. However, mosquito control efforts still rely on chemical-based interventions; (3) an inclusive, regenerative, eco-friendly, and viii sustainable health promotion model is a strong approach to DHF prevention through lemongrass leaf utilization; (4) statistical analysis shows that the health promotion model significantly improves community knowledge, attitudes, and practices regarding DHF prevention. In conclusion, the DHF prevention program in Pacar Keling Village has successfully reduced disease incidence through mosquito control campaigns, abate promotion, and public education. Physical and social environmental factors contribute to DHF transmission, necessitating more sustainable approaches. The health promotion model based on lemongrass leaf utilization effectively enhances public knowledge, attitudes, and practices in DHF prevention, offering a more environmentally friendly alternative to chemical-based methods.
